Registration Requirements
To enroll, the following must be submitted:
1. Completed Online Registration form: Eleyo Registration
2. Non-Refundable Deposit: $25 per child
3. Health Assessment Form
4. Birth Certificate Copy
5. Potassium Iodide Permission Form
Additionally, proof of flu shot is required by December 23
